Stroke ESD Assistant Practitioner

This is a fantastic opportunity to become a part of Calderdale Community Stroke Early Supported Discharge Team.

The successful applicant will have the opportunity to work with a highly skilled group of AHP's and Nurses in the delivery of stroke specific rehabilitation to stroke survivors.

The service covers the whole geographical footprint of Calderdale, with treatment either within the patients home, or a clinic setting.

Main duties of the job

To provide highly skilled support work with people requiring stroke specific rehabilitation.

Working to agreed protocols and guided by the Calderdale Framework of Competency, the post holder will be key in the assessment, treatment, and care of patients and also their carer/relatives where relevant.

The post holder will be working under indirect supervision of the registered practitioners in the team. This will involve taking responsibility for an identified and allocated caseload following screening / assessment by a registered practitioner and will involve a degree of autonomy depending on the complexity and risk of procedures being undertaken.

Work will be carried out in health centres, the service user's own home, residential care, or other community venues. Work may also be completed via video consultation and telephonic discussion.

Monitoring, evaluating and adjusting treatment within the scope of protocols, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required.

Provide individual and group-based education to client groups to support long term condition management.

Assisting registered practitioners with complex treatment

Participate in and deliver education and training of other qualified and non-qualified health and social care staff with support from senior staff.

QUALIFICATIONS / TRAINING
  • 5 GCSEs including Maths and English (grades 9-4 or A*, A, B, C) or equivalent level 2 qualification(s)
  • A-Levels or Health and Science T-Level or equivalent level 3 qualifications
  • Level 5 Assistant Practitioner Apprenticeship / Willingness to complete when in post
  • Foundation degree Health and Social Care
  • Care Certificate / Willingness to complete when in post
  • Band 4 Calderdale Framework Competencies completed / willingness to complete when in post
KNOWLEDGE, EXPERIENCE & EXPERTISE
  • Previous extensive experience as a rehabilitation assistant/ support worker in a health or social care or education field
  • An understanding of the needs and issues concerning illness and disability
  • An ability to work on own and make appropriate decisions, seeking help appropriately particularly in unforeseen circumstances
  • A good understanding of the concept of rehabilitation
  • An understanding of enabling a person to learn or re-learn skills
  • An understanding of the issues when working in service user's own homes
  • Knowledge of the client group (rehabilitation) and evidence of related training
  • Commitment to partake in Level 4 work-based learning education programme
  • Previous experience working in a Multidisciplinary team in health or social care setting
  • Experience of working in hospital, patients' homes and other community settings
  • Experience of teaching and instructing
  • Understanding & knowledge of specialist equipment
  • Understanding of wider interagency issues
