Qualifications

• Current, active Pennsylvania LPN license in good standing.
• Minimum of two years of MDS experience in a skilled nursing facility.
• Knowledge of MDS assessment processes and regulatory requirements.

• Excellent communication and collaboration abilities.

Responsibilities

• Coordinate and complete MDS assessments in accordance with applicable state and federal regulations.
• Ensure assessments are completed accurately and within required timeframes.
• Collaborate with nursing staff, the interdisciplinary team, and other departments to support comprehensive resident assessments and care planning.
• Assist with developing, reviewing, and updating individualized resident care plans based on assessment findings.
• Monitor assessment schedules and maintain accurate documentation.
• Support compliance with CMS guidelines and facility policies.
• Identify assessment-related issues and communicate with the appropriate team members to support timely resolution.
• Maintain organized records and documentation to support regulatory compliance and quality resident care.
