Job summary

UCHealth University of Colorado Health in Aurora, CO is seeking an experienced RN for an on-site Care Continuum Integration role. The position supports outreach and education programs aligned with the service line strategic plan.

The role requires RN licensure, BLS, and at least 2 years of relevant experience. Bachelor’s degree in Nursing preferred. The job offers a full-time schedule with 80 hours per pay period and a pay range of $38.91-$60.31 per hour.

Qualifications

• Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program.
• State licensure as a Registered Nurse (RN).
• BLS for Healthcare Provider (American Heart Association) and any position-level certification as required.
• Minimum of 2 years’ relevant experience.

Responsibilities

• Coordinates and implements outreach and education programs that meet the service line strategic plan.
• Establishes effective professional communication and builds relationships with healthcare and community partners.
• Establishes effective tools and programs of value for targeted stakeholders based on available resources.
• Partners with leadership and system stakeholders to coordinate outreach and educational initiatives according to the plan and objectives.
• Advocates for and promotes the service line in alignment with UCHealth objectives.
• Demonstrates critical thinking and decisive judgment, able to work with minimal supervision in a fast-paced environment.
