Job summary

Jefferson Health in Philadelphia is seeking a Director of Integrated Care Coordination to provide strategic and operational leadership for the Inpatient Care Management Department. This role aligns with hospital and enterprise goals and oversees care coordination across multiple acute care sites.

The director will drive process improvements, regulatory compliance, and data-driven performance improvements to enhance patient transitions, reduce length of stay, and optimize resources.

Qualifications

• Master’s degree in healthcare, management, or related field.
• 3 years of hospital management or care management experience.
• Familiarity with EPIC.
• Strong interpersonal and presentation skills.
• CCM or ACM certification within 3 years.

Responsibilities

• Provides strategic and operational leadership for the care management department, aligning programs with hospital goals.
• Develops and oversees policies and processes for care coordination, discharge planning, and authorization processes.
• Directs and evaluates departmental operations, staffing, budgeting, and performance improvement.
• Collaborates with physicians and leadership to facilitate patient progression and transitions across the continuum of care.
• Ensures regulatory and payer compliance and drives data-driven performance improvements.
• Oversees daily operations, staffing deployment, and departmental effectiveness.
• Monitors productivity and turnover; develops strategies to recruit and retain staff.
