CHA Hollywood Presbyterian Medical Center is seeking an experienced RN Case Manager to lead Case Management, Utilization Review, and Discharge planning in a dynamic acute care setting. The role focuses on coordinating care across the continuum to achieve safe, timely, and cost-effective patient outcomes.

The ideal candidate will have RN licensure in California, 5+ years of case management in an acute care hospital, and comfort working in a union environment.

Competenze

• Ability to communicate effectively verbally and in writing.
• Must be able to work in a union environment.
• Case Management experience in Acute Care Hospital 5 years or more required.

Mansioni

• Supervises Case Management, Utilization Review and Discharge planning activities.
• Oversees throughput and timely discharge of patients.
• Coordinate internal DRG review and contracts with third party payers/hospices as needed.
• Lead performance improvement initiatives and staff evaluations.
• Collaborate with hospitalists and physicians to meet length-of-stay and quality metrics.
• Ensure compliance with organization-wide Performance Improvement Program.
