Qualifications

• Bachelor's degree in Nursing, Healthcare Administration, Public Health, or related field required.
• Minimum of three to five years of clinical care management, population health, utilization management, or Value Based Care experience preferred.
• RN license in good standing; CCM/ACM or equivalent certification preferred.

Responsibilities

• Oversee day-to-day operations of Value Based Care clinical programs to improve quality and efficiency.
• Analyze utilization and population health data to reduce inpatient and ED utilization.
• Develop and monitor population health initiatives for Value Based beneficiaries.
• Coordinate transitions of care and ensure follow-up across providers and services.
• Supervise multidisciplinary care teams and support staff.
• Lead education and training programs; participate in quality committees and provider meetings.
• Build relationships with providers and community partners to align with strategic goals.
• Travel to provider offices and care sites as needed for education and oversight.
