Responsibilities include conducting assessments, developing care plans, coordinating with providers, educating patients and families, and maintaining compliant documentation. Strong communication and organizational skills are essential.

Qualifications

• Active, unrestricted RN license (state unspecified).
• Minimum 3+ years of clinical experience.
• 1+ year of case management experience preferred.

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the State of Florida (must be maintained)
• 3+ years of clinical experience
• 1+ year of case management experience preferred

Preferred Skills & Abilities

• Strong computer proficiency, including Excel
• Experience with Electronic Medical Records (EMR) systems (Athena experience a plus)
• Excellent communication, organization, and problem-solving skills

Compensation Range: $66,675.00 to $111,125.00

The anticipated base salary range represents the Company's good‑faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
