Deercreekseniorliving is seeking a Director of Admissions for Home Health & Hospice to lead patient intake, collaborate with clinical operations and marketing, and drive compliant, efficient admission workflows. The role is hands-on, overseeing referrals, insurance verification, and data entry while mentoring the intake team.

The ideal candidate will have RN or LPN credentials, strong intake experience, and deep knowledge of Medicare/Medicaid rules to ensure a patient-centered onboarding

Qualificações

• RN or LPN preferred.
• Strong background in intake coordination, admissions, or healthcare marketing.
• Knowledge of home health and hospice regulations.
• Experience with EMR systems and insurance verification.
• Understanding of Medicare/Medicaid requirements.
• Proven ability to build referral relationships.
• Leadership, organizational, and communication skills.
• Ability to manage multiple priorities in a fast-paced environment.

Responsabilidades

• Lead and supervise Intake Coordinators and Lead Intake Coordinator.
• Collaborate with clinical admissions, marketing; liaison between teams.
• Coach, train, and manage intake staff.
• Oversee referral intake, verification, and data entry accuracy.
• Ensure timely clinical assessments and admissions.
• Maintain smooth transitions of care for all patients.
• Develop referral relationships with hospitals and physicians.
• Monitor admission metrics and lead process improvement initiatives.
• Ensure regulatory compliance across Medicare, Medicaid, state, and accreditation standards.
• Educate patients and families about services and rights.
