Responsibilities

• Assist residents with ADLs including bathing, dressing, grooming, toileting, and oral hygiene.
• Support residents with mobility and transfers using assistive devices as needed.
• Assist with feeding and monitor nutrition and hydration.
• Record and monitor residents' vital signs and report changes to nursing staff.
• Document care in the facility's EHR or paper charts; maintain accurate records.
• Ensure resident safety; maintain a clean, organized, and comfortable environment.
• Provide emotional support and engage residents in social activities.
