Job summary

Qualifications

• Doctor of Pharmacy Degree OR BS Pharmacy Degree.
• With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required.
• Hospital pharmacy experience preferred.

Responsibilities

• Review and monitor patient medication therapy for safety and efficacy.
• Prepare, label and dispense medications per physician orders and regulatory rules.
• Educate caregivers, patients and students on medications and proper use.
• Maintain records of dispensing and manage medication systems.
