Job summary

CommunityCare, Inc. is seeking a Quality Improvement - Clinical Quality Program RN (STARS) in Tulsa, OK. The role focuses on promoting, protecting, and improving members’ health by leading initiatives to close care gaps and strengthen Medicare Advantage STARS performance.

The RN collaborates with interdisciplinary teams, providers, and community resources to deliver high-quality, cost-effective care and to educate network staff on population health initiatives and best practices.

Qualifications

• Strong clinical assessment, critical thinking, and problem-solving skills.
• Excellent verbal and written communication, including cultural competence.
• Proficiency with EHR systems and clinical software.
• Ability to work independently and in a multidisciplinary team.
• Fluency in English (spoken and written).

Responsibilities

• Monitor and track member data to identify gaps in care (HEDIS/STARS).
• Coordinate services across the continuum of care and ensure transitions.
• Engage members via phone or secure messaging to support self-management.
• Document interactions and analyze outcomes using EHR/population health tools.
• Educate providers and staff on population health initiatives and quality metrics.
• Advocate for patients, addressing social determinants of health and disparities.
• Perform other duties as assigned.
