Nurse Practitioner or Physician Assistant, Bridge Care (Hybrid Schedule/Full-time)

University Medical Partners (UMP), a partner of Stanford Medicine, is seeking a full-time BC/BE Primary Care Nurse Practitioner or Physician Assistant to join our group. Our Bridge Care team provides clinic support and support clinicians by managing in-basket (messages, calls, refills, results etc.) and conducting video visits.

Position Details:

  • Community-based practice model
  • Full time 40 hours per week
  • Clinicians can see up to an average of 10-12 patients per half day
  • 100% on-site to start to build out team but position will transition to a hybrid schedule once comfortable with workflow
  • Opportunity to see patients half day per week in Emeryville Express Care as part of in-clinic time
  • Laboratory and radiology services are available
  • Access to Stanford specialists
  • We utilize EPIC EMR as our electronic medical record
  • Diverse patient population

Qualifications:

  • Minimum 3 years of primary care or urgent care experience is required
  • Board Certified Nurse Practitioner or Physician Assistant
  • An active and unrestricted California Medical License
  • Work with collaborative and supportive staff
  • A positive attitude, flexible, and high energy
