Advocate Aurora Health is seeking a Registered Nurse (RN) for Home Health Case Management in Wauwatosa, Wisconsin.

Job Description & Requirements

Major Responsibilities:

• Manage caseloads of patients in specialty areas.
• Conduct initial assessments, establish care plans, provide direct care, and coordinate with the healthcare team.
• Document assessments, updates, and patient responses accurately and promptly.
• Communicate with physicians and facilitate discharge planning.
• Participate in quality improvement activities and maintain accurate records.
• Provide age-appropriate care, understanding growth and development stages.

Licensure, Registration, and/or Certification Required:

• RN license issued by the state of practice.
• Basic Life Support (BLS) certification within 6 months.
• Valid driver’s license.

Education Required:

• Associate's Degree in Nursing.

Experience Required:

• Typically 3 years of clinical nursing experience.

Knowledge, Skills & Abilities:

• Excellent communication and interpersonal skills.
• Ability to work well with physicians and other professionals.
• Proficiency in clinical skills and judgment.
• Ability to operate necessary equipment and assess patient data.
• Respect for cultural diversity.

Physical Requirements and Working Conditions:

• Manual dexterity, physical mobility, and stamina for patient care and travel.
• Ability to lift, push, pull, and perform transfers as specified.
• Exposure to varying environmental conditions and hazards.
