Advocate Aurora Health is seeking a Registered Nurse (RN) for a Home Health nursing position in Westmont, Illinois.

Part-Time Home Care Liaison - Good Samaritan Hospital, Downers Grove

Hours: Part-time days with weekday and holiday rotation. Schedule typically 2 days one week / 3 days the next.

Major Responsibilities:

• Screening and referral processing to ensure patient eligibility and compliance with regulations.
• Verifying benefits and obtaining authorizations.
• Ensuring physician orders are obtained.
• Identifying risk situations and completing high-risk referrals.
• Interpreting services, policies, and guidelines to referral sources and patients.
• Documenting care plans and initiating communication.
• Assessing patient appropriateness for home care.
• Collaborating with care management to initiate services.
• Responding to inquiries and providing education.
• Following productivity guidelines and reporting accordingly.
• Training patients and families on equipment use.

Qualifications:

• Registered Nurse (RN) license.
• 3-4 years’ experience, with 1-2 years in home care preferred.

Knowledge, Skills & Abilities:

• Excellent communication skills, knowledge of Medicare rules, multitasking, customer service, and critical thinking.

Physical Requirements:

• On-call and weekend rotation, ability to drive in all weather conditions, lift up to 35 pounds.

Compensation: $35.90 - $53.90 per hour
