Advocate Aurora Health is seeking a Registered Nurse (RN) for Home Health Case Management in Olympia Fields, Illinois.

Position: Registered Nurse - Home Health Case Manager

Location: Far Southwest Suburbs including Worth, Alsip, Merrionette Park, and surrounding communities

Hours: Days with weekend rotation

This position offers a sign-on bonus of up to $10,000. Eligibility requires current employment in this role at payout time. Internal team members are not eligible.

Major Responsibilities:

1. Nurse/Patient Relationship: Develops and maintains therapeutic relationships, assesses patient and family needs, and involves them in care planning.
2. Ensuring/Improving Quality Care: Facilitates quality, cost-effective care, mobilizes resources, participates in quality initiatives, and adheres to regulations.
3. Patient/Staff Education: Meets learning needs of patients and families, promotes educational growth among staff.
4. Nursing Process: Applies nursing problem-solving, recognizes clinical changes, prioritizes care, and coordinates referrals and discharge plans.
5. Leadership/Collaboration/Professional Development: Demonstrates teamwork, leadership, and ongoing professional growth, including certifications and contact hours.

Licensure: Registered Nurse (RN) in Illinois

Education & Experience:

• Graduate from an accredited nursing school
• 10+ continuing education credits annually
• At least 1 year of medical-surgical experience

Knowledge, Skills & Abilities:

• Effective communication and documentation skills
• Computer proficiency
• Time management and organizational skills
• Familiarity with patient care equipment
• Current Illinois RN license and CPR certification

Physical & Working Conditions:

• Ability to work flexible hours, including weekends and holidays
• Ability to handle multiple tasks, transport objects and patients, and respond quickly to needs
• Exposure to potentially hazardous conditions and infectious materials
• Ability to lift up to 35 pounds without assistance, with patient handling equipment used for heavier lifts
