Advocate Aurora Health is seeking a Registered Nurse (RN) for Home Health Case Management in Country Club Hills, Illinois.

Position Details

Location: Far Southwest Suburbs, focusing on Worth, Alsip, Merrionette Park, and surrounding communities.

Hours: Days with rotating weekend schedule.

This position offers a sign-on bonus of up to $10,000! Eligibility applies if currently employed in this role at the time of payout. Internal candidates are not eligible.

Major Responsibilities:

  • Nurse/Patient Relationship: Develops and maintains a therapeutic relationship throughout the healthcare continuum, demonstrating adaptive communication, providing individualized care, assessing patient and family readiness, and involving them in care planning.
  • Ensuring/Improving Quality Care: Facilitates quality, cost-effective care, mobilizes resources, adheres to evidence-based standards, participates in quality initiatives, and meets regulatory requirements.
  • Patient/Staff Education: Meets learning needs of patients and families, utilizes resources, and promotes educational growth among staff.
  • Nursing Process: Uses nursing problem-solving models, recognizes clinical changes, makes clinical decisions, prioritizes care, and coordinates discharge planning.
  • Leadership and Collaboration: Demonstrates respect, participates in team activities, maintains current knowledge, and pursues professional development including certifications and degree requirements.

Licensure:

  • Registered Nurse (RN) license in Illinois

Education and Experience:

  • Graduate from an accredited nursing program
  • Minimum 10 continuing education credits annually
  • At least one year of medical-surgical experience
  • Proven proficiency in RN role requirements

Knowledge, Skills & Abilities:

  • Effective communication and interpersonal skills
  • Strong documentation skills
  • Computer literacy
  • Time management and organizational skills
  • Familiarity with patient care equipment
  • Current Illinois RN license and CPR certification

Physical Requirements and Working Conditions:

  • Ability to work weekends, holidays, and various shifts
  • On-call availability
  • Ability to handle multiple tasks quickly
  • Transporting objects, patients, and equipment
  • Responding promptly to patient needs
  • Exposure to potentially hazardous conditions
  • Ability to lift up to 35 pounds without assistance; patient handling equipment used for heavier lifts
