Job summary

Stamford Health is seeking a part-time RN to coordinate patient care using the Nursing Process. You’ll supervise non-RN staff, work mainly from Morgan Street in Stamford with occasional shifts at Darien Pediatrics, and commit to one Saturday per year plus 24–32 hours monthly.

Responsibilities include patient intake, vitals, EMR documentation, minor procedures assistance, patient education, and scheduling/tests coordination. Requires CT RN license and BLS, with at least 2 years’ experience.

Qualifications

• Graduate of an accredited school of nursing.
• Valid CT RN license.
• BLS Certification.
• Knowledge of medical terminology.
• Minimum of 2 years experience in a similar position.
• Must be able to read, write and speak English.

Responsibilities

• Reviews schedules and organizes patient flow and greets patients, documents concerns, and prepares for exams.
• Obtains and documents vital signs and weights, reports abnormalities.
• Assists with specimen collection and testing as needed.
• Documents in EMR and assists with education for patients and families.
• Schedules testing and pre-certifies when required by insurance.
• Maintains clinical rooms and inventories, cleans after procedures.

In this role, you will coordinate and develop patient care utilizing the Nursing Process and provide supervision & mentorship for non-RN team members You will work mainly out of our Morgan Street practice in Stamford however, you may be asked to pick up shifts at our Darien Pediatrics practice as well. You must be able to commit to 1 Saturday shift per year and 3-4 days or 24-32 hours per month (if requested).

Schedule: 8-hour shifts with start times varying between 7:30AM and 9:00AM

Responsibilities:

• Reviews schedules and organizes patient flow; greet patients, documents patients concern, questions, etc. Accompanies patient to exam/procedure room and prepares patients for physical exam. Initial patient interview; reviewing patient medical profile, H&P and document in EMR/manual charting as appropriate
• Obtains and accurately documents patient vital signs, weight and reports any abnormalities to physician.
• Assists with specimen collection and testing as necessary.
• Documents pertinent information in EMR
• Assists patients as necessary i.e. walking, transferring, dressing etc.
• Provides minor treatment as directed by physician, assists physicians with minor procedures
• Provides education as indicated after assessing the learning needs of the patient and or family
• Documents all patient related activities
• As necessary, Schedules patient testing and pre certifies if insurances require
• Responds to all telephone messages (patient, physician offices etc.)
• End of month checks for expired supplies and medication
• Stocks rooms/maintains inventory or exam/procedure rooms
• Orders supplies as needed
• Cleans the exam/procedure room & equipment after each patient
• Prepares exam /procedure room for next patient & physician needs
• Notifies patients of test results (per physician instruction) and follow up
• Attends scheduled staff meetings, in service education opportunities and participates in continuing education programs.
• Formulates and maintains effective working relationships with employees and other managers of the health care team.
• Respects and protects the patient’s rights to confidentiality and privacy and discloses information only for the professional purposes which are in the patient’s best interests with full consideration of their legal rights.
• Displays a positive attitude when interacting with physician’s office staff, physicians and fellow employees of the office.
• Participates in and supports performance improvement and risk management activities
• Performs other related duties as assigned or requested in order to maintain a high level of service.
• Completes required continuous training and education, including practice specific requirements.
• Demonstrates professional work behavior by following Service Standards.
• Complies with departmental organizational policies and procedures and adheres to external agency requirements
