Summary

The role of an Heart Failure/VAD Coordinator is one that encompasses specialized nursing care focused on protection, promotion, and optimization of the health and abilities of the patient with heart failure as well as the Ventricular Assist Device recipient. This position is responsible for working with the multi-disciplinary team to coordinate, oversee, and monitor medical and psychosocial activities related to heart failure; including the collection, organization, and submission of statistical data.

Responsibilities

  • Evaluate VAD recipients and potential recipients by obtaining personal and medical history, reviewing diagnostic and laboratory test results, and scheduling medical/psychosocial consultations as warranted. Assist physicians with medical treatment, and provide plan of care information to patients and/or family members. Schedule clinic visits, labs, diagnostic studies, admissions, and emergency visits, and coordinate patient transport. Provide telephone triage of patient related concerns, reporting medical providers with patient status/condition changes as warranted.
  • Navigate, edit, and maintain the electronic medical record and regulatory electronic data submission elements. Compile, calculate, and analyze statistical, medical, and operational data to report to government, regulatory, and medical organizations. Ensure the accuracy and completeness of regulatory required care plans and organ sharing forms.
  • Provide on-call, Heart Failure/VAD related coverage to address the needs of patients, hospital staff and outside organizations that is facilitated by the use of a rotating call schedule. Act as the liaison with VAD companies. Collaborate with transplant team and advise physicians, patients and/or families regarding eligibility status on the waiting list.
  • Develop and provide education regarding the Ventricular Assist Device maintenance and care. Also provides ongoing education that begins during the initial VAD admission and continues throughout the patient's VAD related care at Children's.
  • Participate in intra and interdisciplinary programs and committees, attend organ selection committee meetings when appropriate, quality assurance performance improvement (QAPI) review meetings, monthly departmental staff education meetings, and participates in orienting fellows/residents.

EDUCATION

  • Four-year Bachelor's degree in Nursing BSN Required

LICENSES AND CERTIFICATIONS

  • Current license to practice professional nursing in the State of Texas Required
  • Certified Heart Failure Nurse (CHFN) within 24 months of employment and must complete continuing education requirements to maintain CHFN Required
  • PALS 1 Year Required
  • BLS Required
