Responsibilities
  • Oversee patient care services and personnel, including assignments, care coordination, referrals, and updating care plans.
  • Plan, direct, and participate in home health services, ensuring timely and compliant delivery.
  • Coordinate physician orders and communication among care team, patients, and families.
  • Review and approve care plans, ensuring clinical appropriateness and integration of physician orders.
  • Conduct regular reviews and team meetings for guidance and coaching.
  • Participate in hiring, training, and performance management of staff.
  • Maintain compliance with regulatory guidelines and organizational policies.
Qualifications
  • Associate degree in Nursing or related field.
  • At least 2 years of home care experience as an RN in Home Health or Hospice.
  • Minimum 1 year of supervisory experience.
  • Current, unrestricted RN license in Texas.
  • Proficiency with enterprise applications such as Homecare Homebase, Workday, and Microsoft Office.
  • Willingness and ability to travel within the coverage area.
  • Dependable vehicle, valid driver’s license, and auto insurance.
  • Physical ability to lift 50-100 lbs and perform related tasks.
