REGISTERED NURSE - HOME HEALTH CASE MANAGER
LOCATION: FAR SOUTHWEST SUBURBS, FOCUSING ON WORTH, ALSIP, MERIONETTE PARK, AND SURROUNDING COMMUNITIES
HOURS: DAYS WITH WEEKEND ROTATION
This position is eligible for a sign-on bonus of up to $10,000! You must be working in this same position at the time of payout. Current internal team members are not eligible.
Major Responsibilities:
- Nurse/Patient Relationship: Develops and maintains a therapeutic relationship throughout the healthcare continuum. Demonstrates adaptive communication styles based on individual patient needs. Provides individualized care to promote physical and psychosocial comfort. Assesses patient and family readiness and involves them in planning and implementing care. Incorporates an individualized, holistic approach in the plan of care.
- Assuring/Improving Quality Care: Facilitates the patient and family’s right to receive quality, cost-effective care. Mobilizes appropriate resources in response to situations that could negatively impact patient and family outcomes. Adapts practice to the latest standards based on evidence. Participates in quality and process improvement initiatives. Works to reduce risk and improve patient safety. Meets regulatory and mandatory requirements.
- Patient/Staff Education: Demonstrates commitment to meeting the learning needs of patients and families. Uses appropriate resources to meet those needs and achieve positive outcomes. Promotes educational growth for nursing peers and colleagues. Assesses patient and family readiness to learn, modifies approach as necessary, and seeks opportunities to enhance staff knowledge.
- Nursing Process: Uses nursing process as a problem-solving model. Recognizes changes in clinical situations, evaluates and implements changes to the care plan. Makes sound clinical decisions, prioritizes and organizes care, and ensures follow-through. Initiates referrals and discharge planning, coordinates interdisciplinary care, and applies ethical decision-making. Ensures confidentiality and proper conduct. Delegates appropriately and seeks resources for disruptive behaviors.
- Leadership/Collaboration/Professional Development: Respects and understands peers and other disciplines. Participates effectively in the care team. Demonstrates leadership and commits to self and others' development. Attends meetings, utilizes resources for current knowledge, and demonstrates effective communication and conflict resolution. Maintains required certifications or a Bachelor's Degree, earning at least 10 contact hours annually.
Licensure:
Education/Experience Required:
- Graduate from an accredited nursing school
- Earn 10 continuing education credits annually
- One year of medical-surgical experience
- Proven proficiency for RN role requirements
Knowledge, Skills & Abilities:
- Good communication and interpersonal skills
- Good documentation skills
- Computer skills
- Time management and organizational skills
- Familiarity with patient care equipment
- Current Illinois RN license
- Current CPR certification
Physical Requirements and Working Conditions:
- Ability to work weekends, holidays, and various shifts
- Ability to take on-call duties
- Handle multiple tasks within short timeframes
- Transport objects, patients, and equipment
- Respond quickly to patient needs and work at a fast pace
- Work environment may include exposure to hazardous conditions and infectious materials
- Ability to lift up to 35 pounds without assistance, with patient handling equipment used for heavier lifts, and assist with patient mobility up to 200 pounds
