Advocate Aurora Health is seeking a Registered Nurse (RN) for Home Health Case Management in Midlothian, Illinois.

Job Description & Requirements

Location: Far Southeast City/Suburbs, focusing on Calumet City, Lynwood, Dolton, Glenwood, Lansing, Posen, South Holland, Thornton, Burnham, Riverdale

Hours: Days; must be able to work one summer and one winter holiday. Must be able to rotate on an on-call schedule.

This position is eligible for a up to a $10,000 sign-on bonus, contingent upon employment at payout. Internal team members are not eligible.

Major Responsibilities:

  • Nurse/Patient Relationship: Develops and maintains a therapeutic relationship, demonstrating adaptive communication, providing individualized care, involving patients and families in planning, and adopting a holistic approach.
  • Assuring/Improving Quality Care: Ensures quality, cost-effective care, mobilizes resources, adheres to standards, participates in quality initiatives, and promotes patient safety.
  • Patient/Staff Education: Meets learning needs, utilizes resources, and fosters educational growth among peers.
  • Nursing Process: Uses nursing problem-solving, recognizes clinical changes, makes clinical decisions, prioritizes care, and coordinates discharge planning.
  • Leadership/Collaboration/Professional Development: Demonstrates respect, participates in team activities, maintains current knowledge, and pursues certifications or degrees.

Licensure:

  • Registered Nurse (RN) license in Illinois
  • Valid driver’s license

Education/Experience:

  • Graduate from an accredited nursing school
  • 10+ continuing education credits annually
  • At least 1 year of medical-surgical experience

Knowledge, Skills & Abilities:

  • Effective communication and documentation skills
  • Computer proficiency
  • Time management and organizational skills
  • Knowledge of patient care equipment
  • Current Illinois RN license and CPR certification

Physical Requirements & Working Conditions:

  • Ability to work weekends, holidays, and various shifts
  • Ability to handle multiple tasks, transport objects and patients, and respond quickly to needs
  • Exposure to potentially hazardous conditions and infectious materials
  • Ability to lift up to 35 pounds without assistance, with patient handling equipment used for heavier lifts
