Advocate Aurora Health is seeking a Registered Nurse (RN) for a Home Health Case Management position in Harwood Heights, Illinois. This is a full-time, ongoing role with 40 hours per week, primarily during daytime shifts.

Key Responsibilities

• Develop and maintain therapeutic relationships with patients and their families throughout the healthcare continuum.
• Ensure quality, cost-effective care by mobilizing resources and participating in quality improvement initiatives.
• Assess patient and family readiness, provide education, and involve them in care planning.
• Apply nursing process for clinical decision-making, prioritize patient care, and coordinate interdisciplinary efforts.
• Demonstrate leadership and collaborate effectively with healthcare team members.

Qualifications
