Job summary

BAYADA Home Health Care in Colorado is seeking a full-time Registered Nurse (RN) to join our home health visit team. The role requires travel across the coverage area and involves skilled nursing and rehabilitative care to help clients remain at home.

You will use comprehensive head-to-toe assessments, manage acute and post-acute conditions, perform medication reconciliation, wound care, and teach patients and families.

Qualifications

• Current Colorado nursing license in good standing.
• At least one year of hands-on adult/geriatric clinical experience in the past two years.

Responsibilities

• Perform head-to-toe assessments and identify subtle changes in patient condition.
• Provide care for acute and post-acute conditions (CHF, COPD, wound care, IV therapy, post-surgical care).
• Triage and manage urgent clinical situations in the home setting.
• Medication reconciliation, teaching, and safety monitoring.

Registered Nurse - Home Health * $10,000 Incentive with a Full Time Commitment * BAYADA Home Health Care is currently seeking a Full Time Registered Nurse (RN) to join our home health visit team. Prior Medicare Certified Home Health experience strongly preferred. This role requires flexibility and the ability to travel throughout the office's entire coverage area. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. How You’ll Make an Impact

• Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
• Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation

Qualifications for a Registered BAYADA Nurse: A current nursing license in good standing in the state of Colorado Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings).
