Elara Caring is seeking a Registered Nurse Case Manager, Hospice in Valparaiso, Indiana. This salaried role supports home-based hospice care, with responsibilities across patient assessment, care coordination, and bereavement support.

You’ll work under the Clinical Team Manager and contribute to a mission-driven team delivering compassionate end-of-life care. The position requires an RN license, at least 1 year of clinical experience, and travel up to 50%.

Qualifications

• Graduate of an accredited nursing program (Certificate/Diploma/Associate/Baccalaureate).
• Current State Nursing License as an RN.
• At least 1 year in a clinical care setting.
• Experience in hospice or home health is preferred.
• Travel up to 50% of the time.
• Valid driver’s license, insurance, and reliable transportation.
• Job requires lifting up to 50 pounds and assist with transfers.

Responsibilities

• Ensure activities align with Elara Caring’s board and Hospice leadership.
• Assess hospice patients and families to identify physical, psychosocial, and environmental needs.
• Gather data on physical, psychological, social, and spiritual factors affecting health.
• Communicate significant findings and changes to the Clinical Supervisor and care team.
• Counsel and involve patient and family in the Interdisciplinary Plan of Care.
• Educate staff on hospice philosophy and pain/symptom management in care settings.
• Provide support at time of death and conduct bereavement assessment.

As a Registered Nurse Case Manager, you’ll contribute to our success in the following ways:

• Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
• Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations.
• Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
• Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
• Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care.
• Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.).
• Provides appropriate support at time of death and perform bereavement assessment.

What is Required?

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse RN
• 1 year of experience in a clinical care setting
• Experience in a hospice or home health environment is preferred
• 50% travel required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Valid driver’s license and insurance and reliable transportation to perform job tasks
