Your Responsibilities

As a Hospice RN Case Manager, you will:

  • Provide patient care across the continuum, managing hospice services with empathy and expertise.
  • Assess, plan, implement, and evaluate individualized care plans for patients.
  • Support patients and families through end-of-life care with compassion and dignity.
  • Collaborate with interdisciplinary teams to ensure seamless care delivery.
  • Maintain accurate documentation and manage multiple priorities effectively.

What You Bring

  • Education: Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate Nursing Program.
  • Licensure: Current Registered Nurse (RN) license in [Insert State].
  • Experience:
    • Minimum of 1 year in a clinical care setting.
    • Hospice or home health experience (preferred).
  • Skills: Strong nursing skills, ability to manage priorities, and dedication to patient-centered care.
  • Transportation: Reliable transportation to perform job responsibilities.
