Job Description & Responsibilities

In collaboration with the interdisciplinary team, provides care coordination services, evaluating options and services to meet patients' healthcare needs and promote quality, cost-effective outcomes. Acts as a consultant to the healthcare team managing specific patient populations, integrating utilization management, quality management, discharge planning, and post-hospital care coordination.

Essential Duties
  • Manage a caseload of 18-20 patients effectively.
  • Assess physical and biopsychosocial needs through clinical assessments and data analysis.
  • Develop and implement care plans in collaboration with patients, families, and healthcare providers.
  • Participate in interdisciplinary meetings and rounds.
  • Initiate discharge planning within 24 hours of admission and update as needed.
  • Collaborate with social workers to address psychosocial needs.
  • Complete Medicare forms and documentation timely.
  • Serve as a clinical consultant on financial and healthcare issues.
  • Participate in quality improvement and compliance activities.
  • Advocate for patients and involve families in care decisions.
  • Educate patients and families regarding discharge plans and healthcare needs.
  • Propose cost-effective treatment options and address barriers to care.
  • Perform InterQual assessments at admission, change of care level, every three days, and discharge.
  • Manage payer communications and authorization documentation.
  • Perform timely clinical reviews and communicate with payers.
Minimum Qualifications
  • Associate’s Degree in Nursing
  • Minimum 5 years of clinical acute care nursing experience
  • At least 2 years of recent case management experience
  • Knowledge of case management principles and healthcare management
  • Strong problem-solving and multi-tasking skills
Preferred Qualifications
  • Bachelor’s Degree in Nursing
Licenses & Certifications
  • Registered Nurse (CA DCA)
  • Basic Life Support (BLS) from American Heart Association
  • Fire Life Safety Training (LA City) within 30 days of hire

The hourly rate range is $54.86 - $89.97. USC considers multiple factors when extending offers, including experience, education, skills, and market conditions.
