Job Summary:
We are seeking a mission-driven Primary Care Physician to join our interdisciplinary team supporting the Program of All-Inclusive Care for the Elderly (PACE). In this role, you will provide comprehensive medical care to a dedicated panel of older adult participants, helping them maintain independence and improve their quality of life.

Essential Duties and Responsibilities:

• Act as the primary care provider for assigned PACE participants
• Deliver direct medical care in clinic, home, and skilled nursing facility settings
• Order, review, and manage medications and treatment plans
• Coordinate care with the Interdisciplinary Team (IDT)
• Support patients through hospital stays and nursing home transitions
• Educate participants and their caregivers about chronic condition management
• Conduct routine and urgent in-office procedures as needed
• Refer participants to specialist providers when appropriate
• Complete accurate, timely documentation using the electronic medical record (EMR) system
• Gain exposure to Value-Based Care (VBC) models and performance improvement
• Support organizational growth through participation in internal initiatives and team huddles

Qualifications:

• MD or DO from an accredited institution
• Board certification in Internal Medicine, Family Medicine, or Geriatrics (preferred)
• Current license to practice medicine in CA
• DEA license required
• Bilingual in Spanish highly preferred
• Experience with older adult populations or geriatric care a plus
• Knowledge of managed care or value-based care is beneficial

Schedule:

• Monday–Friday, with rotating on-call responsibilities (remote)
