This job maintains and ensures complete and appropriate drug therapy in a multidisciplinary pharmaceutical care environment. Prepares, dispenses, and verifies medications ordered for an assigned group of patients, using safe, rational, and cost-effective therapeutic decision-making. Assesses, evaluates, and monitors patient drug therapy; provides drug information, patient education, and other clinical pharmacy services.

Required - Bachelor of Science in Pharmacy
Preferred - Doctor of Pharmacy (Pharm.D)

Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work when it requires significant walking or standing, or working at a production rate pace involving pushing and pulling.
Must be able to stoop, bend, reach, and grab with arms and hands; manual dexterity required.
Work may require travel between facilities and a flexible schedule (e.g., 24/7, weekends, holidays, on-call).
Normal routine involves no exposure to blood, body fluids, or tissue, but may involve emergency care or first aid. Incumbents work with patients with communicable diseases and may enter isolation rooms. There is occupational risk for exposure to hazardous medications and waste.
Because of the healthcare setting, there may be occupational risks related to hazardous medications or waste during receipt, transport, storage, preparation, dispensing, administration, cleaning, and disposal. The risk level may increase depending on job duties.
