This job maintains and ensures complete and appropriate drug therapy in a multidisciplinary pharmaceutical care environment. Prepares, dispenses, and verifies medications ordered for an assigned group of patients, using safe, rational, and cost-effective therapeutic decision-making. Assesses, evaluates, and monitors patient drug therapy; provides drug information, patient education, and other clinical pharmacy services.

Knowledge, Skills, and Abilities (KSAs)
  • Must have computer skills and dexterity required for data entry and retrieval of patient information.
  • Proficient with Windows-style applications and keyboard.
  • Effective verbal and written communication skills, with the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
  • Skills necessary to compound pharmaceuticals, operate technical equipment, etc.
  • Excellent interpersonal skills.
Job Duties
  • Evaluates all physician orders based on a clinical assessment of the patient, including allergies, age, renal function, concomitant drug therapy, lab values, approved dosing guidelines, best practice protocols, diagnosis, and physical clinical status.
  • Ensures the safe and accurate preparation and dispensing of medications, including sterile products and investigational drugs, measured by QA, medication occurrences, competency assessments, and documentation.
  • Manages drug therapy to optimize patient outcomes, measured by intervention documentation, ADR assessment, chart reviews, patient education, and drug information provision.
  • Maintains professional and technical knowledge, licensures, and certifications through continuing education.
  • Manages pharmacy operations in compliance with state and federal regulations.
  • Adapts behavior to the specific patient population, respecting privacy, communication style, and obtaining permissions as appropriate.

Physical and Environmental Demands

Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work when it requires significant walking or standing, or working at a production rate pace involving pushing and pulling.

Must be able to stoop, bend, reach, and grab with arms and hands; manual dexterity required.

Work may require travel between facilities and a flexible schedule (e.g., 24/7, weekends, holidays, on-call).

Normal routine involves no exposure to blood, body fluids, or tissue, but may involve emergency care or first aid. Incumbents work with patients with communicable diseases and may enter isolation rooms. There is occupational risk for exposure to hazardous medications and waste.

Because of the healthcare setting, there may be occupational risks related to hazardous medications or waste during receipt, transport, storage, preparation, dispensing, administration, cleaning, and disposal. The risk level may increase depending on job duties.
