Forum Extended Care Services in Chicago, IL is seeking a Staff Pharmacist for our Northern IL Pharmacy. This PRN casual role focuses on clinical medication review, supervising dispensing, and ensuring regulatory compliance.

The position requires a PharmD or B.Pharm, Illinois license, and experience in long-term care or hospital settings; IV experience is preferred. Weekend/holiday availability and a competitive benefits package are included.

• Pharmacist licensure in the practice state (Illinois).

• IV preparation/dispensing experience preferred; if not, ability to learn.

Responsabilidades

Shift: 2nd Shift (3:00 pm to 11:30 pm)

Qualifications:

What you'll do as a Staff Pharmacist

• Utilize clinical knowledge to analyze medication profiles and new medication orders for appropriateness of therapy, dose, and potential interactions before dispensing to residents of long-term care facilities.
• Supervise dispensing of medication orders in compliance with State and Federal regulations, as well as best practice standards.
• Comply with all applicable company policies, procedures, and patient protocols. Comply with HIPAA and all current government regulations and professional standards respecting patient care.
• Participate in educational and professional programs and/or review professional literature on an ongoing basis to maintain knowledge and competency in current and developing techniques, professional standards, and the services offered by Forum.
• Assist pharmacy managers and lead pharmacy technicians in managing daily workflow of the pharmacy and supervising technician activity.
• Work in a fast-paced 24/7/365 institutional pharmacy environment.

What we need

• Bachelors or Doctorate degree in Pharmacy from an accredited institution.
• Current unencumbered license as a Registered Pharmacist in the pharmacy's state of practice.
• Previous experience in long-term care or hospital pharmacy preferred.
• IV experience preferred. If no experience in IV preparation/dispensing, must be able to learn and demonstrate competency.
• Excellent math, written and verbal communication, and interpersonal skills required.
