Staff Pharmacist FT Rotating Position Summary

The Staff Pharmacist will provide pharmaceutical care services to patients, including oversight, monitoring, preparation, dispensing, and proper documentation of medication therapy. The pharmacist will also provide drug information, patient information, and oversee information and dispensing systems. Additionally, they will ensure compliance with Federal, State, and local laws and regulations, and may advise physicians on medication utilization.

Responsibilities

  1. Review and monitor patients' medication therapy and orders for safety and efficacy, considering specific patient conditions and health status; advise the patient care team of issues or potential improvements to medication therapy and document interventions.
  2. Prepare, label, and dispense medications according to physicians' prescriptions, ensuring compliance with relevant laws and regulations, and maintaining required records.
  3. Ensure medications are available for patients, properly procured, prepared, stored, and maintained.
  4. Supervise technical personnel assisting in preparation, maintenance, and dispensing of drugs and supplies.
  5. Maintain liaison with medical and nursing staff; provide timely information regarding pharmaceutical supplies, drug usage, compatibility, regulations, and standards.

Education

Required: Pharm.D. or B.S. in Pharmacy

Experience

Preferred: Hospital experience

Certification

Preferred: Accredited Case Manager (ACM)
