Key Responsibilities:

  • Conduct comprehensive assessments of residents’ physical, emotional, and cognitive abilities.
  • Develop individualized treatment plans and goals tailored to each resident’s needs and functional abilities.
  • Use various therapeutic techniques and adaptive equipment to improve a resident's ability to perform daily activities.
  • Ensure that the therapy plan aligns with the resident’s overall care plan.
  • Monitor and document the progress of residents throughout therapy sessions.
  • Provide education to residents' families and caregivers about techniques to support the resident’s rehabilitation and safety at home.
  • Ensure that all treatments and interventions adhere to established safety protocols and professional standards.

Requirements:

  • Current and valid Occupational Therapist license in the state of Pennsylvania
  • Basic Life Support (BLS) certification preferred
  • Previous experience in a SNF or geriatric setting is preferred.
  • Ability to create individualized treatment plans.
  • Strong communication and interpersonal skills to work effectively with residents, families, and staff.
  • Compassionate and patient-centered care approach.
