Description

The Clinical Nurse for the Mount Sinai nursing float pool is a Registered Professional Nurse who provides safe, competent, and quality care based on nursing theory and research to a designated group of patients across various units.

Responsibilities

  • Deliver patient care incorporating Relationship Centered Care and the nursing process based on theory, research, evidence-based practice, and organizational standards.
  • Assess and evaluate patient needs, applying critical thinking in care management.
  • Integrate assessment and intervention skills in nursing care delivery.
  • Collaborate with the healthcare team in developing, implementing, and evaluating care plans.
  • Prioritize patient care activities, including teaching, rounding, coaching, and planning discharge, delegating as appropriate.
  • Educate patients and caregivers about their care plans, health promotion, and disease prevention.
  • Communicate effectively with patients, families, and team members.
  • Manage assignments within the Modified Primary Nursing model, utilizing resources effectively.

Patient Experience

  • Model Mount Sinai Relationship Centered Care principles through actions like Admission Welcome, Bedside Shift Report, and Hourly Rounding.
  • Consider patient values and preferences in all aspects of care.
  • Build and maintain therapeutic relationships with patients and families.
  • Use evidence-based practices to enhance patient perception of care.
  • Handle service recovery and escalate concerns appropriately.
  • Demonstrate caring, respect, empathy, and active listening.

Quality and Safety

  • Practice safe, patient-centered care, identifying and escalating barriers.
  • Apply clinical skills crucial to quality and safety.
  • Implement process improvements based on evidence and institutional protocols.
  • Participate in peer review and quality initiatives.
  • Contribute to quality improvement and research activities.
  • Follow high reliability principles to ensure safety.
  • Document practice accurately to support quality initiatives.

  • Perform charge nurse duties as assigned.
  • Coordinate supplies and resources with management.

Qualifications

Licensing & Certification: Current NYS RN license, BLS, and relevant certifications (NRP, ACLS, PALS) depending on department.
