Base pay range

$36.00/hr - $40.00/hr

Job Summary

The Nurse supports and assists in providing a holistic, therapeutic, person-centered Memory Care Program on the Memory Care Unit. The Nurse will achieve and maintain the highest quality of life for each resident with a diagnosis of a disease that causes dementia residing in or under the care of the facility.

Support Duties
  • Commit to continued growth as a knowledgeable and experienced healthcare provider in the area of Alzheimer’s dementia and other diseases that cause dementia.
  • Obtain at least 12 hours of continual education annually, including the required 16 hours of orientation training and other dementia care-specific trainings for direct care staff.
  • Assist in oversight of the Memory Care Aides and CNAs, including supervision and assistance with ADL care, transportation, and non-scheduled activities.
  • Become knowledgeable of each resident’s background, disease, medical and behavioral care needs, and preferences.
  • Utilize communication devices for residents, such as language services, communication boards, and assistive devices.
  • Provide therapeutic, person-centered memory care activities that enhance quality of life, meet program mission and policies, and comply with standards.
  • Encourage family involvement through volunteering and visitation.
  • Maintain a clean, safe, and hazard-free environment.
  • Assist in providing an individualized symptom management tracking system, including development and maintenance of non-pharmacological interventions.
  • Complete all required documentation.
  • Maintain compliance with policies, procedures, and regulations.
  • Provide feedback to the Memory Care Director to monitor program achievement.
  • Be flexible to adapt to residents’ changing needs.
  • Provide emotional support to residents, families, and the team.
  • Perform other duties as assigned.

Shift: 11 AM – 7:30 AM
