Aufgaben

• Assess the needs of patients based on data collected through history observation, physical exam, telephone triage, and analysis or diagnostic data.
• Performs direct patient care: triage, assessment, procedures, treatment, patient education, and follow up instructions.
• Assist patients and providers in obtaining medicinal prior authorizations when required.
• Verifies the patient understands the purpose of the visit.
• Providers preventative Care information matching the patients age, gender, and health status.
• Maintain knowledge of the principles of the skills needed for practical nursing to provide patient care and treatment.
• Knowledge of examination, diagnostic and treatment room procedures
• Ability to interpret, adapt, and apply guidelines and procedures
• Basic Critical Care and Chronic Illness Management
• Required to participate in training opportunities
• Helps promote office efficiency and encounter volumes
• Ability to understand and administer vaccines and immunizations to pediatric and/or adult patients
• Responsible for daily clinical workflow which includes but not limited to: Referral management, telephone encounters, lab results, medication refills, lab Q&A, compliance with sample medication logs
• Willingness to learn about AIDS Service Organizations and Community Health Centers
• Assist in training new nurses to the organization in the standard working procedures of HopeHealth
• All other duties assigned

Education And Experience

Physical Requirements

Hearing: Adequate to perform job duties in person and over the telephone. Speaking: Must be able to communicate clearly to patients in person and over the telephone. Vision: Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens. Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements (especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
