Job description

Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

Job Role Summary

The Staff Nurse T-I PRN clinical care and uses coordination skills for every patient served. This includes understanding of the expected patients served in the unit'(s) nursing care is delivered using the Eskenazi Health Model of Care-Synergy, the ability to coordinate with the patient/family and other healthcare disciplines to ensure care is appropriate to meet patient needs. Proactively contributes to Eskenazi Health's mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County; models Eskenazi Health values.

Essential Functions and Responsibilities
  • Promotes a healthy work environment for all that provide care while encouraging collaboration for excellence in patient care outcomes.
  • Effectively performs all nursing functions in an efficient manner and in accordance with policies, procedures, the Indiana Nursing Practice Act, the Eskenazi Health Model of Care-the Synergy Model demonstrating synergy characteristics: clinical judgment, advocacy, caring practices, collaboration, systems thinking, response to diversity, clinical inquiry and facilitation of learning.
  • Establishes and/or revises priorities and documents the role/patient care and activities based on the acuity/priority of the need, resources available, customer/patient preference/request/age and other scheduled procedures for patients.
  • Utilizes and documents the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of patient care as evidenced by client/staff observation; care acknowledges physiological, psychological, spiritual, diversity and cultural needs.
  • Provides patient care which reflects nursing intervention, patient response to care provided, patient needs, problems, capabilities, limitations, and progress toward goals and ensures coordination to the next level of care.
  • Teaches patient/significant other appropriate health information in a timely manner and shares written informational material as evidenced by feedback and observation; documentation is reflected in the medical record.
  • Demonstrates ability to handle emergency situations in a prompt, precise, and professional manner.
  • Participates in own professional development by maintaining required competencies, identifying learning needs, and seeking appropriate assistance or educational offerings.
  • Provides personal and individualized patient care to provide comfort and well-being for the patient acknowledging physiological, psychological, spiritual, and cultural needs.
  • Integrates cultural/population-specific diversity needs into patient care.
  • Rounds consistently within the shift to all patients or areas covered to assess care and service; utilizes the Care Rounding process.
  • Provides timely feedback on any customer service issue to the right person/department & participates objectively in the review of customer service issues and service recovery in a timely fashion with the unit/department leadership.
  • Maintains safe patient care and advocates for patient safety through participation in Care Rounds with patients and families throughout the shift care is delivered.
  • Participates in identification of clinical or operational performance improvement opportunities and in performance improvement activities using evidence-based practice.
Job Requirements
  • Current Indiana license as a Registered Nurse
  • RN degree from a nationally accredited college or university
  • Current BLS certification; other certifications may be required in specific units to practice
  • External hires or rehires > 3 years from last employment: Must have 2 years of recent relevant experience (defined as having worked in the last year from time of application) in the desired area of employment.
  • Internal transfers: Must be a Staff Nurse II and have experience in the area of employment. Employees must be in good standing and have a recommendation from the current leadership team.
COMMITMENT
  • Tier I: • 12 hours/pay period
  • • No weekend or holiday requirements
  • • Open to internal applicants only or former employees who have been gone from the organization for no more than 3 years

  • Effective communication skills incorporating listening, effective interviewing techniques with patients and staff appropriate to age of the patient

  • Knowledge of the principles of growth and development over the life span and possesses the ability to assess data/interpret appropriate information needed to identify each patient's requirements relative to age‑specific needs
  • Uses age‑appropriate communication and cognitive skills detailed in the department/area/unit job‑specific/age‑specific competencies
  • Fundamental nursing theories, current clinical concepts, diseases/symptoms, trauma, treatments/medications, triage, appropriate diagnostic/therapeutic procedures and interventions
  • Functions as a member of a health care team, delegate responsibilities/duties appropriately
  • Follows oral/written instructions, records/interprets information accurately
  • Competency in assessing, responding to patient status or needs, and interpreting results
  • Recognizes and reacts to changes in patient status, documents and reports changes
  • Performs CPR and other technical skills
  • Positions patients
  • Must be able to wash hands thoroughly and frequently
  • Must be able to help evacuate patients via stairwells
  • Critical thinking skills are essential
