Job summary

Community First Solutions in Mason, Ohio is seeking a Staff Nurse LPN/RN to deliver direct bedside care, administer medications and treatments, and mentor caregivers to ensure safe, person-centered resident care that honors each individual's needs and goals.

You will collaborate with physicians and the interdisciplinary team to develop and implement care plans, document accurately, supervise RA's and STNA's, and uphold a mission-driven culture that values compassion, teamwork, and ongoing

Qualifications

• Active Ohio nursing license (LPN/RN) required.
• Ability to pass medications and manage treatments.
• Experience in long-term care/geriatrics preferred.
• 12-hour shift availability and weekend rotation.

Responsibilities

• Collaborate with physicians and interdisciplinary team to develop care plans.

• Provide direct leadership and supervision to RA's and STNA's.

Job description

At Community First Solutions, our nursing team takes a resident-first approach-ensuring wellness plans are guided by each individual's needs and goals. As a Staff Nurse LPN/RN, you'll work side-by-side with our caregivers, providing direct bedside care, administering medications and treatments, and collaborating with physicians and the interdisciplinary team to support residents' health and well-being. You'll also guide and mentor caregivers, ensuring a safe, supportive, and person-centered environment.

Schedules: Full Time, 7:00am - 7:00pm, 36 hours per week, every other weekend and holiday rotation

Pay: Starting at $29.00/hour, based on date of state certification.

Responsibilities

• Collaborate with physicians and other interdisciplinary team members to develop, implement and manage resident-driven care plans
• Administer medications and treatments as ordered by Medical Director or Resident physician
• Provide direct leadership, supervision, and support to RA's and STNA's
• Document pertinent information following established clinical policies and procedures
• Effectively communicates with residents, family members, and co-workers
• Protects the health and safety of residents

Qualifications

• Must have an active license in the state of Ohio with authorization to pass medications
• Ability to work 12-hour shifts
• Current Instructor-Led CPR Certification (or willing to obtain upon hire)
• Previous experience in long-term care/geriatrics preferred.
• Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care
• A positive attitude and passion for working with the elderly
• Flexible schedule with the ability to work every 3rd weekend and holidays.
• This direct care position requires COVID-19 vaccination to protect our team and our clients. All applicants who would like to be considered for this position but are not yet vaccinated will be provided resources to become fully vaccinated.
