Job summary

Main Line Health seeks an Ambulatory Registered Nurse for the Downingtown, PA Primary Care practice. In this role you’ll perform clinical triage, care coordination, patient education, and medication refill management to support a seamless patient care experience.

You will join a team focused on preventive care and coordination across lifespan, with a Monday–Friday schedule, 40 hours per week, no weekends or holidays. Requires PA RN license and BLS.

Qualifications

• Experience in ambulatory clinical nursing settings preferred.
• Strong telephone triage experience preferred.
• Experience with care coordination and patient education preferred.

Responsibilities

• Provide clinical triage and ongoing patient communication to support care plans.
• Coordinate care and follow-up for preventive services and chronic conditions.
• Educate patients on medications, refill processes, and health maintenance.

Position:

Registered Nurse RN – Ambulatory Primary Care

Shift:

40 hours per week | Day shift. Monday through Friday from 8AM to 4:30PM | No weekends or holidays!

Experience:

Preferred:

• 1. Experience in an ambulatory clinical nursing setting, primary care practice, family medicine, internal medicine, and/or outpatient office environment.
• 2. Strong telephone triage experience and comfort supporting patients with acute concerns, chronic condition management, preventive care needs, and follow-up care coordination, strongly preferred.
• 3. Experience with medication refills, care gap closure, patient education, care coordination, and knowledge of insurance-related processes preferred.

Education:

Graduate of an accredited ASN or BSN nursing program.

Licensures/Certifications:

• 1. Current Pennsylvania RN license.
• 2. Provider BLS (American Heart Assoc/American Red Cross) required.
