Sr. Clinical Nurse (Allegany, Frederick, and Washington Counties)

The Center for Substance Use and Infectious Disease Care Integration is seeking a full-time Sr. Clinical Nurse to work with our patients in Allegany, Frederick, and Washington Counties.

The Sr. Clinical Nurse is an integral part of a multidisciplinary team working with clinicians, Community Outreach Coordinators, Peer Coaches, and rural Maryland Health Departments in Western Maryland harm reduction teams to engage and treat persons who use drugs and related infectious diseases through care coordination, nursing assessments, and nurse case management services.

This role involves coordinating patient care through the continuum of HIV/HCV/SUD screening, diagnosis, education, linkage to telemedicine, on-treatment support, and follow-up. Responsibilities include onsite and remote medical case management and coordinating treatment delivery in rural health departments. The ideal candidate is self-motivated, highly organized, works independently and in teams, and thrives in a public health environment to connect with diverse populations.

The Clinical Nurse will support projects focused on engagement and wellness of people who use drugs, including those re-entering communities from incarceration, requiring travel. Mileage reimbursement is provided at federal rates. The primary responsibility is ensuring accessibility of long-acting injectable medications (LAIs) used for OUD and HIV prevention at designated telemedicine sites.

Specific Duties & Responsibilities

  • Provide nurse case management services to patients via in-person or telephone contact.
  • Conduct comprehensive nursing intake, document in EMR, coordinate labs, and schedule telemedicine appointments.
  • Collaborate with RNs, NPs, MDs, and Community Health Outreach Workers.
  • Manage LAI medications, validate inventory, ensure proper storage, and communicate with pharmacies.
  • Track prescriptions, lab tests, coordinate administration, and document in EMR.
  • Provide wound care and perform venipuncture.
  • Follow up with patients for medication education, adherence, appointments, referrals, and social determinants of health.
  • Counsel on harm reduction, disease prevention, and treatment plans.
  • Coordinate with pharmacy, insurance, and authorization activities for medication access.
  • Monitor lab and diagnostic results to ensure treatment protocols are followed.
  • Advocate for patients and coordinate with insurance and pharmacies.
  • Promote the program through various outreach methods.
  • Maintain communication with providers and agencies for care coordination.
  • Provide onboarding, training, and logistical support to community partners.
  • Attend meetings at Johns Hopkins in Baltimore as needed.
  • Provide clinical care within the scope of a registered nurse in Maryland.
  • Other duties as assigned.

Additional Knowledge, Skills & Abilities

  • Self-motivated with minimal supervision.

The position requires a registered nurse licensed in Maryland, with a Bachelor's Degree in Nursing or related field, and at least two years of nursing experience. Reliable transportation is necessary. Additional education or experience may substitute for formal requirements.

Preferred qualifications include proficiency with Epic EMR, data management skills, knowledge of HIV/HCV/SUD management, and interest in public health.
