Job summary

Lima Memorial Health System in Lima, OH seeks a Spine & Orthopedic Care Coordinator to manage referrals and coordinate care across spine and orthopedic service lines. You will ensure timely communication among patients, providers, and care teams and support patients through the care journey.

The role requires nursing or medical assistant training, OH LPN license or MA certification, and BLS. 1–3 years' experience in a clinical setting is preferred.

Qualifications

• Graduation from an accredited school of nursing or completion of an accredited Medical Assistant program.
• Valid unrestricted LPN license for Ohio or MA certification is required; BLS is required.
• One to three years' experience in physician office practice or hospital setting preferred.
• Excellent customer service, communication, and problem-solving skills.

Responsibilities

• Coordinate and manage all referrals for spine care and outgoing referrals for orthopedics.
• Follow clinical protocols and adapt to patient and provider requests for first point of care.
• Ensure timely and effective communication among patients, providers, and care teams.
• Monitor care of patients sent outside the health system and support their journey.
