Sweetwater Care is seeking a licensed Speech-Language Pathologist to provide evaluation, treatment, and plan of care for patients with speech, language, swallowing, and cognitive disorders in a SNF setting in Reedley, CA.

The role emphasizes coordination with physicians and therapy colleagues, adherence to Medicare/Medicaid guidelines, and delivering evidence-based therapies to improve intelligibility and swallow safety.

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment planning.
• Provide professional treatment for speech, language, hearing, swallowing and cognitive disorders using therapeutic techniques.
• Plan activities and provide therapeutic services for speech and language disorders.
• Advocate for patients and participate in quality assurance and customer service activities.
• Evaluate patients and develop treatment plans with short- and long-term goals.
• Implement care plans and progress patients according to the care plan.
• Demonstrate knowledge of Medicare/Medicaid and other reimbursement systems.
• Develop treatment plans and obtain approvals from referring physicians.
• Participate in care planning and utilization review meetings.
• Provide SNF-appropriate patient care.
• Administer standardized speech/language assessments and swallow evaluations.
• Perform tests and complex treatments with or without assistance.
• Collaborate with OT/PT and physicians.
• Document evaluations, progress notes and discharge summaries timely.
• Provide a full range of speech therapy modalities.
• Assist with clinical affiliations of speech therapy assistants.
• Perform clinical assessments to identify patient needs.
• Document and prepare Plans of Care and progress notes.
• Ensure delivery of quality rehab services.
• Demonstrate courtesy and respect to patients and staff.
• Maintain compliance with the Code of Conduct.
• Collaborate on case management with facility and organizations.
• Other duties may be assigned if needed.
