Senior Care Solutions in Boca Raton, FL seeks a compassionate Speech Therapist (ST) to provide high-quality home health services on a PRN/Part Time basis. You will work one-on-one with patients to address communication, swallowing, and cognitive-linguistic needs in their homes, with flexibility and clinical support from a collaborative team.

The role offers autonomy, field independence, and a strong emphasis on patient-centered care within Medicare guidelines.

Qualifications

• Active Florida Speech Therapist License.
• 1+ year of home health experience.
• Strong clinical assessment and communication skills.
• Reliable transportation, valid driver’s license, and auto insurance.
• Ability to work independently and manage a PRN caseload.

Responsabilités

• Conduct comprehensive speech, language, cognitive-linguistic, and swallowing evaluations.
• Develop individualized treatment plans based on evaluation findings and patient goals.
• Provide skilled therapy interventions, including communication therapy, swallowing therapy, and cognitive retraining.
• Educate patients and caregivers on home exercises, safety, and communication strategies.
• Document all care in compliance with Medicare and agency standards.
• Communicate regularly with nurses, therapists, and other interdisciplinary team members to ensure coordinated care.
• Report any significant changes in patient condition to the physician and clinical team.

Overview: We are seeking a compassionate and skilled Speech Therapist(ST) to provide high-quality, patient-centered speech therapy services on a PRN/Part Time basis in the home health setting. In this role, you will work one-on-one with patients to address communication, swallowing, and cognitive-linguistic needs—helping them regain function and improve their quality of life from the comfort of their homes.

This role offers flexible scheduling, independence in the field, and the support of a collaborative clinical team.
