Job summary

West Village Post Acute in Greenville, SC is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis.

You will evaluate and treat residents with communication, cognitive, and swallowing disorders while collaborating with physicians, nursing staff, families, and other healthcare professionals to help residents achieve their highest level of function and quality of life.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology required.
• SC SLP license in good standing; state licensure requirements met.
• Experience in skilled nursing or rehab preferred; PCC/Casamba experience preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide speech, language, cognitive, and swallowing therapy services.
• Monitor progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff, residents and families on goals.
• Complete accurate and timely documentation per policies and regulations.
• Participate in care planning and discharge planning activities.
• Educate residents and families on treatment plans and maintenance.
• Maintain compliance with state practice acts and facility policies.
• Promote resident safety, independence and quality of life.
