Job description

This position is for an Independent Contractor to serve in Covina area specifically.

FeldCare Connects is currently seeking a self-motivated Speech Therapist to deliver excellent care and work with adults of all ages.

The Speech Therapist in Home Health is responsible for assessing and evaluating patient needs related to speech and language disorders, communication, swallowing, and cognitive impairments. Based on this, the therapist develops a treatment plan, implements interventions to improve patient well-being, and monitors progress.

A Speech Therapist for Home Health must:

1. Interpret, plan, establish, and execute treatment programs based on physician referrals.
2. Document and report patient responses and condition changes.
3. Educate and supervise patients, families, and healthcare team members regarding therapy procedures.
4. Make recommendations considering the home environment and individual needs.
5. Maintain detailed records of each visit in the medical record.
6. Review treatment responses and advise physicians on ongoing or discontinued therapy.

Qualifications:

• Enthusiasm and ability to work with all ages.
• State licensure and registration as a Speech Therapist.
• Completion of an accredited Speech Therapy program.
• Certificate of Clinical Competence (CCC) in speech pathology or audiology from the American Speech-Language-Hearing Association.
• Bilingual skills are a plus!
