Qualifications

• Experience with cognitive and dysphagia rehabilitation in SNF preferred.
• Experience with Rehab Optima preferred.
• Strong clinical, communication, organizational, and documentation skills.

Per Diem Speech-Language Pathologist (SLP)

Location: Lilac Manor – Rochester, NY

Position: Per Diem

Compensation: $58–$62 per hour

Lilac is seeking a dedicated and experienced Per Diem Speech-Language Pathologist (SLP) to join our rehabilitation team. This role offers the opportunity to provide meaningful, patient-centered care in a supportive skilled nursing environment while working collaboratively with an experienced interdisciplinary team.

Responsibilities

• Evaluate patients and provide appropriate speech-language pathology services based on individual clinical needs.
• Conduct screenings, initiate referrals, gather relevant clinical information, and develop individualized plans of care.
• Provide treatment for communication, cognitive, and swallowing disorders.
• Complete timely and accurate documentation, including evaluations, progress notes, daily treatment notes, and discharge summaries.
• Educate patients, families, and facility staff regarding treatment goals, progress, and therapeutic techniques.
• Collaborate with nursing, therapy, medical, and interdisciplinary team members to support positive patient outcomes.
• Maintain compliance with facility policies, professional standards, and applicable state and federal regulations.

Qualifications

• Valid New York State Speech-Language Pathologist license.
• Certificate of Clinical Competence (CCC) required.
• Experience with cognitive and dysphagia rehabilitation in a skilled nursing facility setting preferred.
• Experience with Rehab Optima preferred.
