Job summary

Otterbein SeniorLife in Tipp City, Ohio is seeking a compassionate Speech Therapist to join our home health team, delivering care in patients’ homes per individualized plans. As a licensed Speech-Language Pathologist you will conduct evaluations, set goals, implement therapy, coordinate with physicians and caregivers, and document progress in a timely manner per state and Otterbein guidelines.

• One year of supervised professional experience in providing Speech-Language Pathology services.
• Meets the education and experience requirements for the ASHA CCC.

Responsibilities

• Health Home in Tipp City region with minimal travel to Lebanon campus for training.
• Conducts evaluative procedures, assesses home environment, and identifies equipment needs.
• Identify functional speech defects, establish goals, and participate in care plan development.
• Administer speech therapy program per plan of care.
• Recommend communication devices/aids as indicated.
• Coordinate with attending physician and home health team to promote coordinated care.
• Participate in meetings and in-services; refer to other home health services as needed.
• Supervise homemakers/home health aides during therapy regime.
• Prepare and submit clinical and progress notes in a timely manner.
• Maintain clinical competency in speech-language pathology practice and theory.
• Instruct patient/family/caregiver and team members in treatment regime.
• Perform other duties to facilitate service delivery.
• Complete a minimum of twelve hours of in-services per year.
• Meet or exceed Otterbein Home Health service standards.
• Interact positively with staff and support the mission.
• Comply with laws, Medicare regulations and Otterbein policies (HIPAA).
• Adhere to Code of Conduct.

• Health Home in Tipp City region with minimal travel to Lebanon campus for training.
• Conducts appropriate evaluative procedures, assesses patient’s home environment as appropriate, and identifies patient’s equipment needs relative to speech/language function.
• Identify functional speech defects, establish goals to improve patient’s speech function and participate in the development of the team care plan.
• Administer speech therapy program utilizing specialized therapeutic techniques and and/or equipment in accordance with the plan of care.
• Recommend communication devices/aids as indicated.
• Confer with the attending physician, home health team members and patient and/or family/caregivers to promote coordinated and comprehensive care.
• Participate in meetings, in‑services and other related activities as needed.
• Identify patient and family needs for other home health services and refer them to the services.
• Supervise homemakers /home health aides when they are participating in the patient’s therapy regime.
• Prepare and submit clinical and progress notes in a timely manner and in accordance with Otterbein Home Health, state and federal guidelines and regulations.
• Maintain clinical competency in speech language pathology practice and theory.
• Instruct patient and/or family/caregiver and other team members in the patient’s treatment regime as indicated.
• Perform other duties as required to facilitate the delivery of speech language pathology services and swallowing assistive services.
• Complete a minimum of twelve (12) hours of in‑services per year.
• Meet or exceed delivery of Otterbein Home Health service standards in a consistent fashion.
• Interact with all staff in a positive manner, supporting the Otterbein Home Health Mission.
• Comply with local, state, and federal laws, Medicare regulations and established Otterbein Home Health company policies and procedures (which includes confidentiality and HIPPA).
• Conduct all business activities in a professional and ethical manner and adheres to Otterbein Home Health’s Code of Conduct.

Education

• Must possess a master’s degree from an accredited program in Speech-Language Pathology.

Experience

• One year of supervised professional experience in providing Speech‑Language Pathology services.
• Meets the education and experience requirements for a Certificate of Clinical Competence in Speech‑Language Pathology, granted by the American Speech‑Language‑Hearing Association (ASHA).

Licensure, Registration, and/or Certification

• Must possess current license as a Speech Language Pathologist from the State of Ohio or made application for same.
• Must possess valid driver’s license.
• Must have the use of an automobile in good working condition and automobile insurance.
• Satisfactory criminal background check.
• Maintains current CPR certification.
