Job summary

Otterbein SeniorLife seeks a dedicated Physical Therapist to provide home health services to older adults in the West Liberty area. You will assess patients, develop and implement therapy plans, and educate patients and families on exercises and care strategies, with a focus on improving mobility and quality of life.

The role involves part-time or PRN hours, local travel across Auglaize, Logan, Champaign, and surrounding counties, and collaboration with an interdisciplinary care team to deliver

Qualifications

• Bachelor's or Master's degree in Physical Therapy.
• Minimum 2 years of Physical Therapist experience; home health preferred.
• Current Ohio PT license and CPR certification; valid Ohio driver’s license; reliable transportation.

Responsibilities

• Provide physical therapy services per plan of care.
• Assess muscle strength, mobility, gait, ROM, and transfer capabilities.
• Instruct patients, families, and caregivers in therapy exercises and use of appliances.
• Report changes to physician; adjust care as needed.
• Initiate therapy programs and educate staff and family.
• Supervise PTA as necessary.
• Participate in quality assessment and performance improvement.
• Coordinate care with attending physician and home care team.
• Submit clinical and progress notes timely and per guidelines.

Otterbein Home Health provides Home Health services for the elderly, complimenting already strong independent senior services, outpatient capabilities and clinical facilities at Otterbein. This position travels locally throughout the greater West Liberty Oh area (Auglaize, Logan, Champaign, and Hardin counties) providing patient services in a Home Health care setting. Providing a high level of personalized care, Home Health by Otterbein is guided by Otterbein's nearly century-old tradition of faith-based ministry, clinical skills and strong community ties. The Physical Therapist provides rehabilitative care (physical therapy) for home health patients needing assistance and training in ambulation, transferring, gait training and home exercise programs. The Physical Therapist works as part of the interdisciplinary home care team including the Registered Nurse, Physical Therapist, Speech Therapist, Social Worker and Home Health Aide. Shift: Part Time and PRN

Responsibilities

• Provide physical therapy services to patients according to a plan of care. This may include assessing and evaluating therapeutic/rehabilitative/functional status and participation in developing the total plan of care as well as providing direct physical therapy treatment.
• Assess patients for muscle strength, mobility, gait, ROM and transfer capabilities.
• Instruct patients, families and caregivers in the use and care of therapeutic appliances.
• Report patient’s reaction to treatment or changes in care to patient’s physician as needed and appropriate.
• Initiate physical therapy programs and instruct other personnel, patient and/or family/caregivers in their therapy program, as well as the goals of therapy.
• Supervise the Physical Therapist Assistant (PTA) as necessary.
• Participate in Otterbein Universal Home Health Quality Assessment/Performance Improvement program.
• Confer with the attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care.
• Prepare and submit clinical and progress notes in a timely manner and in accordance with Otterbein Universal Home Health, state and federal guidelines and regulations.

Education And Experience

• Education: Bachelor's degree or master's in physical therapy.
• Experience: Minimum 2 years of physical therapist experience with community / home health preferred.
• Licensure, Registration, and/or Certification: Current license as a Physical Therapist from the State of Ohio or made application for same. Must have a valid Ohio driver’s license and access to reliable transportation. Current CPR certification required.
• Complete a minimum of twelve (12) hours of in-services per year.
