Responsibilities

The Speech Therapist in Home Health assesses and evaluates patient needs related to speech and language disorders, communication, swallowing, and cognitive impairments. Based on this, they develop treatment plans, perform interventions to improve patient well-being, and monitor progress.

Services are provided within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.

The therapist evaluates the patient's functional status, body systems, and speech therapy needs using standardized tools, and consults with physicians to develop the therapy plan.

Treatments include addressing speech production, fluency, language, cognition, voice, resonance, swallowing, and auditory habilitation/rehabilitation, following evidence-based standards.

The therapist observes, records, and reports patient responses and condition changes to supervisors or physicians.

They instruct patients, families, caregivers, and other healthcare team members in therapy areas within the scope of speech-language pathology.

Visits are conducted across the service area and nearby agencies as needed.

Progress notes are completed after each visit and submitted according to policy. Evaluations are completed, and plans of care are developed within state guidelines, with outcomes, goals, and projected care frequency reported within 24 hours of evaluation.

Education and Experience

License Requirements:

  • Current license in Speech Therapy
  • Current CPR certification
  • Valid driver's license and vehicle insurance
