Qualifications

• CFY internship completed by date of hire.
• Graduate with ASHA-certified Speech-Language Pathology degree or foreign trained equivalent.
• California SLP license required at hire.
• National Provider Identifier required at hire.
• Basic Life Support certification required at hire.
• ASHA Clinical Competence certificate required at hire.

Responsibilities

• Deliver Direct Patient Care under the provisions of licensure acts and Kaiser Permanente HealthPlan.
• Provide accurate and timely documentation of services per state and federal requirements and policies.
• Participate in multidisciplinary clinics, patient care rounds, and case management conferences.
• Refer patients to appropriate medical providers when care is outside of scope of practice.
• Instruct patient education classes and/or groups as requested by management.
• Provide department in-service and clinical instruction as requested by management.
• Mentor staff and assist in orientation and training of employees as needed.
• Participate in performance improvement projects or other committees as requested by management.

Education

Graduate of an accredited speech language pathology curriculum with a Bachelor or Master or Doctoral degree with certification in Speech Language Pathology or foreign trained equivalent
