The Speech Therapist (ST) uses clinical expertise, physical assessment, teaching and skilled therapy interventions to deliver high quality patient care in the patient's residence. The ST develops or contributes to the development of the home health plan of care under the direction of the physician and facilitates interdisciplinary involvement to ensure the best patient outcomes. The ST delivers therapy interventions as directed by the physician, consistent with clinical best practices, leading to high quality, improved outcomes, and exceptional patient experience.

Patient Care/Care Coordination
Education: Master's degree in speech-language pathology; Doctoral preferred.
Experience: At least one year in a similar patient population; home care experience preferred (2+ years).
Licenses/Certifications: State licensure as a speech-language pathologist; valid driver's license and insurance; CPR/BLS certification; Child & Dependent Adult Abuse training; Person Centered Care course within 12 months of hire.
Preferred: Certification in home care relevant programming.
Knowledge of medications, interpersonal skills, teamwork, policy adherence, basic Microsoft Office skills, reliable transportation, and experience with electronic health records preferred.
Ability to exert up to 50 pounds occasionally, 20 pounds frequently, and 10 pounds constantly.
