Speech Therapist/Pathologist

PRN; work as needed

Patients are seen Monday-Friday, 8am-4:30pm (No holidays, call or weekends!)

The Speech Therapist (ST) uses clinical expertise, physical assessment, teaching and skilled therapy interventions to deliver high quality patient care in the patient's residence. The ST develops or contributes to the development of the home health plan of care under the direction of the physician and facilitates interdisciplinary involvement to ensure the best patient outcomes. The ST delivers therapy interventions as directed by the physician, consistent with clinical best practices, leading to high quality, improved outcomes, and exceptional patient experience.

Responsibilities

Patient Care/Care Coordination

  • Make home visits to patients requiring speech therapy, conducting ongoing interdisciplinary assessments.
  • Perform initial and comprehensive assessments per agency policies.
  • Identify dysfunctional language, swallowing, cognition, and memory deficits.
  • Oversee treatment modalities and communication aids as needed.
  • Develop speech therapy procedures based on evaluations and include them in the plan of care.
  • Ensure quality and safe delivery of services according to the plan.
  • Use equipment and supplies safely and efficiently.
  • Promote patient independence through education on treatment, medications, equipment, emergency preparedness, and the care plan.
  • Provide services ordered by the physician, including counseling and education for patients, caregivers, and families.
  • Develop and update the plan of care, including discharge planning and patient and family involvement.
  • Maintain accurate documentation, relay verbal orders timely, and communicate with physicians and healthcare team.
  • Identify risks for hospitalization or emergency visits and intervene proactively.
Home Care Performance Criteria
  • Deliver quality care with proper documentation, participate in quality reviews, and maintain infection control procedures.
  • Participate in peer review, quality assurance, and team meetings.
  • Maintain professional knowledge, communicate effectively, and foster positive relationships.
  • Assist with new employee orientation, organize work, and meet productivity standards.
  • Contribute to quality outcomes and perform other duties as assigned.
Qualifications

Education: Master's degree in speech-language pathology; Doctoral preferred.

Experience: At least one year in a similar patient population; home care experience preferred (2+ years).

Licenses/Certifications: State licensure as a speech-language pathologist; valid driver's license and insurance; CPR/BLS certification; Child & Dependent Adult Abuse training; Person Centered Care course within 12 months of hire.

Preferred: Certification in home care relevant programming.

Skills and Abilities

Knowledge of medications, interpersonal skills, teamwork, policy adherence, basic Microsoft Office skills, reliable transportation, and experience with electronic health records preferred.

Lifting Requirements

Ability to exert up to 50 pounds occasionally, 20 pounds frequently, and 10 pounds constantly.
