Northern Counties Health Care, Inc. is seeking a compassionate Speech Therapist to join our home health team and deliver high-quality care directly in patients' homes.

You will work autonomously while staying connected with an interdisciplinary group to support patient goals and comfort. Responsibilities include evaluating speech-language function, planning and implementing therapy, documenting progress, recommending devices, and collaborating with physicians to ensure well-coordinated care for

Qualifications

  • Master's or doctoral degree in speech-language pathology; Vermont licensure.
  • Two years of home health or community-based experience preferred.
  • Current CPR certification; valid driver's license.

Responsibilities

  • Evaluate speech-language function and home environment; identify equipment needs.
  • Develop and implement plan of care to improve patient function.
  • Administer speech therapy programs using specialized techniques/equipment.
  • Recommend communication devices as indicated; ensure coordinated care with physicians.
  • Provide documentation per policy; participate in case conferences and team meetings.
  • Maintain clinical competency and educate patient/family in treatment plans.
  • Identify needs for other services and make referrals as appropriate.
  • May supervise home health aide in therapy-related tasks.
  • Perform additional duties to support services.

Job Summary:

The Speech Therapist is responsible for implementation of standards of care for speech-language pathology services and for adherence to all conditions in the Service Agreement.

Supervisory Responsibilities:

This position has no direct supervisory responsibilities.

Essential Job Functions/Responsibilities:
  • Conducts appropriate evaluations, assesses home environment (as appropriate), and identifies equipment needs relative to speech/language function.
  • Identifies functional speech defects and establishes a plan of care to improve patient’s function.
  • Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
  • Recommends communication devices/aids as indicated.
  • Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.
  • As appropriate, participates in case conferences, in-services and other meetings required to ensure coordinated and comprehensive care.
  • Prepares and submits documentation per organization policy.
  • Maintains clinical competency in speech-language pathology practice and theory.
  • Identifies patient and family/caregiver needs for other services and refers as appropriate.
  • Instructs patient, family/caregiver and other Organization health care personnel in the patient’s treatment regime as indicated.
  • May supervise home health aide in procedures directly related to the speech-language therapy plan of care.
  • Performs other duties as required to facilitate the delivery of speech-language pathology services.

Position Qualifications:
  • Possesses a master’s or doctoral degree in speech-language pathology (SLP), and is licensed as a speech-language pathologist by the State of Vermont
  • Two years of appropriate experience as a Speech-Language Pathology, community/home health experience is preferred.
  • Possesses and maintains current CPR certification.
  • Complies with acceptable professional standards and practice.
  • Excellent observation, verbal and written communication skills, problem solving skills.
  • Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
  • May occasionally be in a position to lift and/or carry an item weighing up to 50 pounds.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
